　　我們，下列簽署人：

　　We, the undersigned,

　　Chen [ ], female, born on [ ] (Date), residing at No. [ ], [ ] Rd [ ] City, [ ] Country.

　　Lee [ ] , female, born on [ ] (Date), residing at No. [ ], [ ] Rd [ ] City, [ ] Country.
